List your preferred lobby performance date(s) and how many people will perform. Include dates and names of each concert. Lobby performance requests are not guaranteed and are subject to approval by CSA staff.(Required)

Example: Sunday, September 14: An Afternoon with the Colorado Cello Ensemble. Quartet (2 violins, 1 viola, 1 cello)
